A big income year can show up in your Medicare premium later.

IRMAA is the income-related extra on Part B and Part D, on a two-year lag. Why it matters before you make other Medicare moves.

Until AEP opens October 15, 2026

This counts down to when the federal review window opens. It is not a sales deadline. You can review and stay.

Medicare Part B has a standard premium. Some people pay more because of income. That extra is IRMAA. It can apply to Part D too. Social Security usually withholds it. It uses tax data from two years ago, not last Tuesday.

A Roth conversion, a business sale, a large capital gain, or a year you worked extra can raise what you pay for Medicare later. People are surprised because the mailer that enrolled them never mentioned it.

What this is not

It is not a reason to skip Medicare you need. It is not a DIY tax plan in a blog post. I am a broker and a Registered Social Security Analyst, not your CPA. If IRMAA might apply, the move is to know it exists before you time income and before you pick a coverage build.

Life events (marriage, retirement, work stoppage) can sometimes justify a new determination. That is paperwork with a reason, not a complaint call.

Why I put this in September

Because AEP conversations ignore income until January’s bill feels wrong. If your income jumped, say so at the review. It may not change Advantage vs Supplement. It may change how you think about timing. Bring the question. We will tell you when it is a CPA problem vs a coverage problem.
